Age and handling do most of the damage here: the cap gets gripped whenever the neck is separated, and a hairline split eventually runs far enough that the piece drops away, leaving the socket open to whatever the head picks up off the floor. Once that happens the connection stops being held square, and a brush roll that cuts in and out as the neck flexes usually follows.
