Part CI358W is a white inlet valve for the Nutone 0613 central vacuum system, the wall plate you open to plug in the hose. It is listed specifically for the 0613, and its white shell blends into most trim and wall colors instead of drawing the eye to the connection point.
Inlets fail inside the wall while the rest of the system stays fine, which is why a weak-suction complaint often traces back to a valve nobody suspected.
